Concierge Enrollment Fee
The patient’s Concierge enrollment fee is paid starting the day the agreement is signed. This may be paid yearly or semi-annually (reference chart below). Cost of Concierge plan is slightly higher if not paid in one lump sum annually, see below for details.

The Concierge Enrollment Fee is not a covered benefit under any insurance company including Medicare, so insurance companies will not pay for the fee—it is the sole responsibility of the patient.
The Concierge enrollment fee is not in lieu of insurance. You will still need health insurance to cover medical costs not included in the annual fee. Also, your existing coverage will be used for care you need outside of my office with specialists, hospitals, labs, x-rays, or pharmacies.

The annual fee may be paid through employer Section 125 plans, and is compatible with flexible spending accounts (FSAs), medical savings accounts (MSAs), and health reimbursement accounts (HRAs). Depending upon the type of plan in which you participate, Dr. DeBruin will be happy to provide you with a statement for your benefits administrator at your request.

In addition to the Concierge membership fee, all Concierge members will continue to pay for medical services as they do in the current fashion. The Concierge fee may change with advance notice to the patient.
Payment Policies:
Concierge Members of DeBruin Medical Center have full access to the doctor from the day they register and pay the enrollment fee.

Initial payment is due upon registering as a Concierge member in the practice and can be paid with check, cash or credit card (Visa, MasterCard, American Express and Discover are accepted).

A Late Fee of 10% of the semi-annual fee will be applied to the statement for the month following the late payment.

A $25 Returned Check Fee will be assessed.

Cancellation Policies:
The agreement renews automatically each year, but the patient or the doctor may cancel the Concierge membership in our program at any time. We will return any unused membership fees to you less a small administrative charge. Any past-due fees owed to DeBruin Medical Center upon a cancellation are still owed to the practice. Debruin Medical Center reserves the right to cancel this agreement for nonpayment of fees.
